with the subject “Application Form Payment – Full Name”. Please provide the following details in the body of the email: full name, mobile number, name of previous university + major taken.

Submit the Following Required Documents to Admissions Office

• A completed application form with correct and valid information
• 2 (two) pieces of 4×6 color photographs
• 1 (one) copy of identity card (KTP or passport)
• 1 (one) certified copy of undergraduate diploma
• 1 (one) certified copy of undergraduate transcript (for applicants who graduated from overseas universities, the copy of transcript must be legalized first by DIKTI)
• 1 (one) copy of birth certificate
• Latest resume/curriculum vitae (CV)
• Receipt of application fee payment

For International Students, Please Also Submit The Following Additional Required Documents

• 1 (one) copy of valid passport (first page only)
• Financial guarantee statement
• Legal and employment declarations
• Medical certificate issued by a doctor/hospital
• Medical insurance which can be claimed in Indonesia
